The Westmoreland County Transition Council is a network of parents, educators, and community service providers helping students with special needs in their transition from school to adult life.  

 

Our mission is to coordinate and communicate available resources and supports, educating the local community, youth, families, educational staff and related agencies in Westmoreland County.

​

Individuals with disabilities have unique needs and sometimes require outside assistance.  In Westmoreland County there are many agencies that are there to help.  Go to the Community Services page to learn more about the agencies in our area.  

 

Feel free to contact the Council if you need additional help locating an agency to address your need, and we will try to help.

There are many tools available to help families navigate the path of transition planning. Visit the Parent and Student resource page to explore topics such as post-secondary education, employment supports, and independent living supports.  

Educators play an important role in

assisting youth with disabilities in preparing for their transition from school to life after school. The Educator Resource page has information to assist educators as they guide youth through the transition process.
